Findings: The resident had medication orders dated August 6, 2025, as well as additional orders dated August 12, 2025. A review of the resident’s Medication Administration Record (MAR) for the period of August 1 through August 31, 2025, indicates that scheduled medications were not administered, as they had been placed on hold. Furthermore, the resident had other medications that were not on hold, yet were also not administered. The facility’s failure to administer medications in accordance with the prescribed orders was substantiated. An investigation concluded that this constitutes a violation of the Oregon Administrative Rules.
