- Findings
- Resident returned the facility with required assistance with ambulation and transferring. Resident service plan dated on or about August 16, 2025 indicated resident is independent with all mobility and transfer needs. The facility failure to ensure the service plan reflected the resident's needs was substantiated. An investigation determined this is a violation of Oregon Administrative Rules.
