- The facility failed to update resident profiles at least quarterly in an Acuity-Based Staffing Tool (ABST). On 10/14/2025, a review of the facility's ABST report indicated that 52 of 61 residents had not been evaluated quarterly. The facility’s failure is a violation of Oregon Administrative Rules.
