Findings: Based on interview and record review, conducted during an investigation on 12/03/25, the facility’s failure to provide records to the Department upon request was substantiated. Findings include but are not limited to: A review of electronic communication, dated 10/16/25, indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event timeframe from 09/21/25 through 09/27/25. The facility had not provided all requested documentation by the due date of 10/17/25. The facility's failure is a violation of Oregon Administrative Rules.
