- Findings
- Based on observation, interview, and record review, the facility failed to take reasonable precautions to ensure the health, safety, and welfare of residents related to smoking in the building while using oxygen and catching fire and not positioning properly during meals resulting in choking. This constituted an immediate threat to resident’s health and safety. The facility’s failure is a violation of Oregon Administrative Rules.
