Findings: Based on interview and record review, conducted during a site visit on October 8, 2025, the facility’s failure to ensure an accurate Medication Administration Record (MAR) is kept of all medications was substantiated for 1 of 1 sampled resident (#1). Findings include, but are not limited to: Refer to LCU00094743 Medication Administration Record (MAR) dated August 2025, indicated staff had signed off that they administered Resident 1's Olanzapine four times while the facility did not have the medication. Interview with Staff 2: - Staff 5 said the charting of Olanzapine administration to Resident 1 was in error; and the medication was unavailable and was not administered The facility’s failure to ensure an accurate Medication Administration Record (MAR) is kept of all medications is a violation of Oregon Administrative Rules.
