- Findings
- The facility failed to ensure medication and treatment orders must be carried out as prescribed in accordance with OAR 411-054-0055(1)(f). Per complainant, a resident missed Amiodarone-cardiac medication on 12/28/25, 12/30/2025 and 1/1/2026. AV takes it every other day 200mg in the morning. The resident missed Levothyroxine-thyroid medication on 12/31/2025 75mg one tablet daily in the morning, which is a violation of Oregon Administrative Rules.
