Findings: On or about August 11, 2025, Alleged Victim had been found by a family member at approximately 11:12 AM laying in his/her recliner breathing heavily, 911 had been called, and resident was sent to the hospital. The last time the Alleged Victim had been seen by staff had been 8:12 PM the night prior on 08/10/25. The MARs indicated the Alleged Victim had not received his/her medication on 08/11/25. Facility Staff confirmed the Alleged Victim had not received any medications. The facility allegedly failed to administer medication as ordered for the Alleged Victim. An investigation determined this is a violation of Oregon Administrative Rules.
