Findings: The facility failed to have qualified awake direct care staff, sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident to include supervision and support in accordance with OAR 411-054-0070(1); Per complainant, resident slipped out of h/her recliner and was uninjured. Staff said they had several people injured and unable to lift and that other staff members are busy completing other tasks at other locations in the building. The facilities failure to provide appropriate staffing is a violation of Oregon Administrative Rules.
