Findings: Based on interview and record review conducted during the investigation on April 22, 2026, the allegation that the facility failed to provide records to the Department upon request was substantiated. Findings include, but are not limited to, the following: A review of electronic communication dated September 3, 2025, showed that the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) for the alleged event dated August 25, 2025. The facility did not provide all requested documentation by the due date of September 4, 2025. The facility's failure to provide records to the Department upon request is a violation of Oregon Administrative Rules.
