Findings: Based on interviews and a review of facility records conducted during the investigation on or about May14, 2026, it was determined that the facility failed to conduct required background checks on multiple recently hired staff. There was no documented evidence that background checks had been initiated for the three identified staff members, as required prior to the start of their employment. On May 08, 2026, at approximately 11:29 p.m., Staff 3 (Caregiver) was found alone and unsupervised with Resident 1 in the resident’s room, during which Staff 3 allegedly sexually assaulted Resident 1. The allegation that the facility failed to submit background check requests for all subject individuals prior to the start of employment was investigated and determined to be substantiated. This constitutes a violation of Oregon Administrative Rules.
