Findings: Based on interviews and a review of facility records conducted during the investigation on 05/22/26, the allegation that the facility failed to provide records to the Department upon request was substantiated. Findings include, but are not limited to, the following: A review of electronic communication dated 04/09/26 showed that the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) for an alleged event occurring on 04/02/26. The facility did not provide all requested documentation by the required due date of 04/10/26. This failure to submit the records as requested is a violation of Oregon Administrative Rules.
