- Findings
- Based on interview and record review, conducted during an investigation on 05/28/26, the facility’s failure to provide records to the Department upon request was substantiated. Findings include but are not limited to: A review of electronic communication, dated 10/11/24, indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event date 09/13/24. The facility had not provided all requested documentation by the due date of 10/14/24. The facility’s failure to provide records to the Department is a violation of Oregon Administrative Rules.
