Findings: On or about April 26, 2026, Resident 2 reported feeling uncomfortable after Staff 8 (Caregiver) touched his/her hair. Staff 3 reported the concern to Staff 6 (Residential Care Coordinator), but no follow-up occurred and no protective measures were documented. The concern remained uninvestigated until a second allegation surfaced on or about May 19, 2026, involving inappropriate physical contact between Staff 8 and Resident 3. Staff confirmed that the initial report was never forwarded to the Administrator, who stated they were unaware of it until after the second incident. The Respondent failed to investigate reports of abuse and suspected abuse and take measures necessary to protect residents and prevent the reoccurrence of abuse. An investigation determined this is a violation of Oregon Administrative Rules.
