- The facility failed to ensure medication and treatment orders must be carried out as prescribed, which resulted in Resident 1 receiving the incorrect medication and then being admitted to the ICU. An investigation determined this is a violation of Oregon Administrative Rules and caused serious risk of harm to the resident.
