Findings include but are not limited to: A review of electronic communication, dated 5/26/2026, indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event date 5/17/2026. The facility had not provided all requested documentation by the due date of 5/27/2026. The facility’s failure to provide records to the Department upon request is a violation of Oregon Administrative Rules.
