Findings include but are not limited to: A review of electronic communication, dated 05/26/26, indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event date 05/16/26. The facility had not provided all requested documentation by the due date of 05/27/26. The facility’s failure to provide records to the Department upon request is a violation of Oregon Administrative Rules.
