Findings: Based on interview and record review, conducted during an investigation on 08/18/26, the facility’s failure to complete or update and review the Acuity-Based Staffing Tool (ABST) evaluation for each resident quarterly, as required, was substantiated. Findings include, but are not limited to: - The ABST was reviewed for the alleged event dates of 08/04/26 through 08/11/26. - A review of the facility’s last ABST evaluation update per resident indicated 6 of 43 residents had not been updated quarterly as required. The facility’s failure to complete or update and review the Acuity-Based Staffing Tool (ABST) evaluation for each resident quarterly, as required, is a violation of Oregon Administrative Rules.
