- The facility neglected the Alleged Victims (AVs) care as defined in OAR 4110200002(1)(b)(A)(i) by failing to provide an adequate medication administration system resulting in AV not getting AVs medications, including behavior medications, as prescribed resulting in risk of serious harm due to AVs increased behaviors.
