Findings: Facility Selfreport received on 5/30/2018 at 8:16am stated that: Neglect of care: Facility failed to provide a safe medication administration system Note: Abuse was suspected on 5/13/18, facility investigation was completed soon after. Facility administrator new to position, needed assistance with proper reporting procedures to APS and proper steps for investigation. Delay of facility reporting did not put RV in an unsafe situation as RVs safety was being addressed by facility staff and delay in reporting was not intentional.
Sanction: ALFCP18-192 $1125.00 fine assessed
