- The facility failed to develop an initial service plan prior to the resident movein that meets the residents' needs, which is based on the initial resident evaluation conducted in person prior to resident move in, as required by OAR 4110540030 (3)(c) and 4110540036 (2)(a)(A). Per a complaint that the facility did not develop an initial service plan that addressed resident's needs, history of falls, and depression prior to move in.
