Findings: On 7/14/21 at 9:05 am, the surveyor observed caregiving staff provide incontinent care to Resident 5. During the observation, Staff 7 (CG) failed to change gloves after removing the soiled incontinence product and wiping fecal matter from Resident 5's bottom and perineum. Staff 7 touched the resident's clothing, new incontinent brief, and applied barrier cream to the resident's bottom while wearing the same soiled gloves; Multiple care staff were observed during meals not wearing aprons or performing hand hygiene prior to serving residents meals or assisting residents with eating.
