- Based on record review and interview it was confirmed that facility failed to update the resident ' s service plan quarterly. Findings include but not limited to: A review of Resident #1 (R1)'s service plan indicated it was last updated on 4/3/2022. Staff #3 was in agreement with these findings. Facility POC: S3 to schedule service planning meeting with resident and family for the week of 8/22/2022-8/26/2022.
