Findings: During onsite record review on 6/28/2022 Compliance Specialist (CS) found the schedule indicated there should have been one med aide (MA) and two caregivers (CG) on duty. On 6/19/2022 there was one MA and a front desk worker with CG competency working. Interviews of Staff #1-2 (S1-S2) state that on 6/19/2022 there were only two staff members working for a four-hour period and knew with were short staffed. S2 states that the call light system is on a pager and a close by facilities frequencies can mess up their system on occasions. This making it hard to know how long call lights have been going off for. CS interviewed Resident #1 (R1) stated that the facility is understaffed and had to wait on hour after pushing their call light.
