Findings: Based on observation and interview it was confirmed that the facility failed to keep all interior materials and surfaces clean. Findings include but not limited to: During an unannounced site visit on 09/08/2022, Compliance Specialist (CS) toured facility and noticed a black film on the shower floor as well as a wet and crumbly spill at the base of a recliner in Resident #3 (R3)'s room.
