- Based on observation, record review and interview the facility failed to take measures to prevent the entry of bed bugs in the facility. Findings include: During site visit on 10/20/2022 Compliance Specialist (CS) observed bed bugs in Resident #2 room on the fourth floor. CS observed what appears to be bites on Resident #2 legs that they state are from bed bugs. CS observed dead bed bugs Resident #2 had collected from their room. In separate Interviews on 10/20/2022 with Staff #1-4 (S1-4), they stated the following: S1 stated they treat for bed bugs in house with Crossfire spray first and if that doesn’t' work, they will use a " baker " Convectex Bed Bug Heater to treat the area. S1 stated that they do not use an outside pest control agency because " they don ' t work ". They stated the previous owner hired an outside agency over three years ago. S1 states they have not done education for residents, conducted town hall meetings or sent notices to residents concerning bed bugs in facility. S1-4 stated the bed bug problem had been going on for more than three years without and resolution. S3-4 stated a resident on the fourth floor had a bed bug outbreak that was treated a week ago, this resident has also had previous outbreaks that had been treated but the bed bugs came back. S2-4 stated a resident on the fifth floor had bed bugs that were treated 3-4 weeks ago with spray but the resident refuses to let anyone back in to check for bed bugs or clean. S2-4 stated a resident on the second floor had a bed bug outbreak 2-3 weeks ago and was treated with Crossfire spray. S3-4 stated the resident ' s mattress were not replaced after bed bug outbreak mattress was just covered with zippered mattress covers. S4 stated that they can only check rooms for bed bugs if resident allows housekeeping in their room, "most residents refuse to let us clean their rooms or change their sheets so we can ' t check for bed bugs". S3-4 stated some rooms on the fourth and fifth floor had reports of bed bugs in the last 3 months, but the residents refuse to let us in. S1-4 stated they do not keep logs of cleaning or treating for bed bugs. S4 stated that they will use Crossfire spray bottles to treat the affected areas let dry for two hours than vacuum and will treat again if the bugs comeback they will use the " baker " to treat the resident ' s room. S3-4 stated they have had bed bug bites as a result of working in the facility. S4 stated that they only purchase the Cross fire when they have an outbreak, and they don ' t currently have any on hand. They only treat when they find bed bugs. In separate interviews with Residents #1-2 (R1-2) they report the following: R1 stated they had a bed bug outbreak in their room on the second floor a month ago, it was treated with spray only not the heater unit " baker ". They report this was not the first time they had bed bugs, 4-6 months ago their room was treated for bed bugs. R2 stated they have had bed bugs for almost a year. They report that they have seen bed bugs in other resident ' s rooms and in the hallways. They stated they have reported the problem to Administrator and written letters to the building ' s owners with no response to the issue. They report that they have bites on their body from bed bugs. In an interview with W1 over the phone on 10/20/2022 they stated that they observed bed bugs and bites on a resident ' s legs from the fourth floor of the facility. CS reviewed the following: Amazon receipts for Crossfire deliveries on 11/12/2021 of 3 bottles (MGK 1852 concentrate), 06/08/2022 of 1 bottle (MGK 1852 concentrate), 07/14/2022 of 3 bottles (MGK 1852 concentrate), and 09/23/2022 of 3 bottle (MGK 1852 concentrate). Bed Bug print-out for 2nd floor dated 10/19/2022, that indicates rooms 202, 203, 205, 206, 208, 210, 212, 213, 215, 216 and, 219 were checked for bed bugs. " Draft " Forest Glen Rapid Response Bed Bug Treatment and Prevention Protocol dated 09/28/2020 that states if treatment fails furniture and mattress are to be disposed of, must treat the rooms with heater between 135-140 degrees with an air mover, and they will provide education and notification to residents. Environmental Services Standards dated June 2009 and, description of Convectex Bed Bug Heat Equipment purchased 10/07/2020. On 10/20/2022, these findings were reviewed with and acknowledged by S1.
