- Findings
- On or about April 18, 2024, the facility was using the Department's ABST and resident census was 57. A review of the facility's ABST and resident roster indicated there were six residents not entered into the ABST and a total of 22 residents whose ABST had not been updated since 04/2023, 09/2023, and 10/2023. The day shift with highest total of care time was 39.75 hours, swing shift highest total care time was 32.3 hours and night shift with the highest total care time was 7.25 hours. A review of the facility's staff schedule, dated April 2024, indicated the facility consistently staff with two MTs on day and swing shifts, four CGs on day shift, three CGs on swing shift, and one MT and one CG on night shift. A review of the Resident 16's records and his/her ABST profile, indicated service plan, dated 04/04/24, indicated s/he in the area of "ambulation/transfer" needed assistance of "two-three person assistance with all transfers am, pm, noc, and prn [as needed]. Uses a 3 person transfer if resident complains of extreme weakness." There was no evidence Resident 16's ABST profile included the "additional staff" needed to perform the duties as care planned. Resident 16 stated staff transfer him/her with one staff person using a gait belt safely. S/he has been participating in physical therapy and feels safe in the community and with the assistance of one staff person for transfers. In separate interviews on 04/16/24 and 04/17/24, Staff 5, Staff 6, Staff 7, Staff 9, Staff 10, Staff 14, Staff 15, Staff 17, and Staff 23 stated there were no residents who required two-person transfers, Resident 16 had gotten stronger doing PT, and s/he could transfer with the assistance of one staff person. It was confirmed the facility failed to fully implement and update an Acuity Based Staffing Tool (ABST).
