- The facility failed to provide qualified awake direct care staff sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident in accordance with OAR 411-054-0070(1) per complaint that staff take more than 45 minutes to respond to call lights, and the resident often has to make multiple call requests to receive assistance with toileting, which is a violation of Oregon Administrative Rules.
