Findings: Based on interview and record review, conducted during a site visit on 08/18/25, 08/20/25, and 08/21/25, the facility 's failure to visually observe the resident take the medication was substantiated for 1 of 1 sampled resident. Findings include, but are not limited to: An incident report, dated 03/25/24, indicated Resident 1 ' s medications had been found in his/her room on 03/24/24. On 08/21/25, Staff 6 stated s/he had found Resident 1 ' s evening medications on the floor of his/her room on 03/24/24. Refer to staffing allegation OR00038804. It was determined the facility failed to visually observe the resident take the medication. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules.
