- The facility failed to have a training program that includes methods to determine competency of direct care staff through evaluation, observation, or written testing. Facility shall also maintain documentation regarding each direct care staff ' s demonstrated competency according to a complaint staff do not know how to properly transfer residents, which is a violation of Oregon Administrative Rules.
