The findings of the kitchen inspection, conducted 02/07/23, are documented in this report. The survey was conducted to determine compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services - Meals and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

The findings of the first revisit to the kitchen inspection of 02/07/23, conducted 04/24/23 are documented in this report. It was determined the facility was in substantial compliance with the OARs 411-054-0030 for Residential Care and Assisted Living Facilities for Resident Services- Meals, and Oregon Health Service Food Sanitation Rules OARs 333-150-0000.

Based on observation and interview, it was determined the facility failed to ensure the kitchen was clean in accordance with the Food Sanitation Rules OARs 333-150-0000. Findings include, but are not limited to:

On 02/07/23 at 10:55 am, the kitchen was observed to need cleaning in the following areas:

* Ceiling vents and light fixtures between the stove and steam table had heavy building up of dust/grease;

* Areas of the dropped ceiling above and in front of stove and steam table had heavy dust buildup;

* The wall behind and above the beverage station had dust/debris buildup:

* Dropped ceiling above the clean side of the dish washing area had dust/debris buildup; and

* The corner near the ceiling by the delivery door had buildup of cobwebs containing dead insects.

* Dishwashing area:

 - The top of the dishwasher, heater booster, and garbage disposal located under the spray hose faucet sink had buildup of crumbs/food debris and splatters; and

 - Wall behind the spray hose faucet sink from the corner to the dish machine had buildup of black/brown matter.

* Stove area:

 - Vents above the stove had buildup of grease/dust;

 - Sides, front and behind stove had drips/splatters;

 - Food drips between stove and counter;

 - Food splatters on wall above the stove;

 - Shelf below deep fat fryer had food debris;

 - Counter under and wall behind steamer had food debris.

* Three, three-tier carts had food debris on shelves;  

* Back of toaster (near steam table) had buildup of dust and grime;

* Microwave (near steam table) had crumbs on top of it and buildup of dust and grime behind it;

* Plate warmer holder had buildup of food debris;

* Counter around the steam table had crumbs/food debris;

* Rolling cart holding desserts had sheet pan shelf on the bottom heavily soiled with debris and food buildup; and

*Bottom storage shelves throughout the kitchen had food debris/crumbs.

Three garbage cans did not have lids on when not in use.

Containers of powdered sugar and dry cereal in dry storage room had scoops/cups in them.

The areas which required cleaning were observed and discussed with Staff 1 (Executive Chef) on 02/07/23. The findings were acknowledged.

(1) Kitchen has been provded with a daily and weekly cleanig schedule. It will be montiored by Dietary Manager / ED

(2)ED / Dietary Manager will oversee the kitchen locations that were sited and ensure all is clean and on cleaning rotation.
