Findings: Alleged Victim (AV) had a history of falls and was a fall risk. AV was showing signs of confusion and forgetfulness. The previous interventions that were in place to mitigate AV's risk of injury due to falls was discontinued on or about January 6, 2019. On or about January 10, 2019, AV walked out of the facility unassisted and fell while walking down the road causing injury to his/her face. AV was taken to the hospital for treatment. The facility failed to implement interventions for AV's known risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
