Findings: The facility failed to ensure supervision and staff support regarding known behaviors related to Witness#1 (W1). An incident occurred between W1 and the Alleged Victim (AV) where W1 swung an article of clothing at AV multiple times, hitting him/her in the face. The facility failed to provide 1 to 1 care when necessary as outlined in the care plan. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
