Findings: Alleged Victim (AV) suffers from chronic pain and is prescribed as needed pain medications. AV reports that he/she did not get his/her as needed pain medication on several occasions. Between January 1, 2019 through March 31, 2019, staff recorded forty one (41) doses of AV's as needed pain medication on the narcotic log however there was not a corresponding entry of the medication administration record indicating the medication had been given, causing AV unreasonable discomfort. The facility practice was to periodically audit the medication administration records and narcotic log to discover errors, but the facility failed to do so consistently. The facility did not provide the investigator with training documents for staff. The facility failed to provide a safe medication administration system which is a violation of resident rights, is neglect of care and constitutes abuse.
