Findings: On or about October 31, 2019, the Alleged Victim suffered a fall and was transferred to the hospital and diagnosed with a broken leg. The facility failed to follow AV's care plan to have non-slip socks on AV to aid in fall prevention. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
