Findings: The facility failed to provide adequate supervision and implement appropriate interventions according to Witness 4’s (W4) known increasing aggressive behaviors. W4 was care planned to be monitored and eyes on his/her when out of his/her room. On or about September 30, 2019, Alleged Victim (AV) and W4 were involved in an altercation, resulting in W4 pushing AV’s wheelchair backwards into a table. Staff was present but did not have eyes on W4 at the time of the incident. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00400 $500.00 fine assessed
