Findings: The facility failed to provide a safe environment by having staff present when the Alleged Victim (AV) is in the common area to redirect other residents from him/her to prevent altercations. Witness #1 and AV had an altercation, neither resident was hurt in the altercation, however, the altercations place the residents at a risk for harm. W1 and the AV were known to have altercations in the past. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
