Findings: The facility failed to follow Alleged Victim's (AV) care plan regarding his/her risk of falls and failed to ensure AV was not left alone when ambulating. An investigation determined that AV experienced three (3) unwitnessed falls on October 30, 2019, November 23, 2019, and December 2, 2019. AV's fall on December 2, 2019 resulted in a laceration to AV's scalp and AV was sent to hospital for treatment. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
