Findings: The facility failed to provide a safe environment for the Alleged Victim (AV). On or about January 23, 2020, AV was found on the floor with his/her arm twisted in his/her walker and skin tears on his/her right arm and cheek. Witness #1 was in AV's room, causing an altercation. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
