Findings: On or about February 1, 2020, Alleged Perpetrator 2 (AP2) administered a discontinued medication to Alleged Victim (AV) after AP2 was directed not to administer the medication. AP2's actions are considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
