Findings: The facility failed to provide a safe environment for the Alleged Victim (AV) and to provide a working call system ensure resident safety. On or about January 31, 2020, AV fell in his/her room and was unable to call for help as his/her call pendant was not working. AV was on the ground from a fall from approximately 2:00 am until 6:00 am when staff finally found him/her, this is the second time that AV has fallen and his/her call light was inoperable. AV suffered rug burns and a bruise to his/her jaw. The facility's failure to provide a safe environment and to have a working call system is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00604 $500.00 fine assessed
