Findings: The facility failed to appropriately care plan and follow the care plan for the Alleged Victim regarding his/her risk of falls. AV was care planned to use his/her 4 wheeled walker while ambulating. On or about February 12, 2020, AV was not using the walker and fell, causing a dislocated shoulder. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
