Findings: Witness 1 (W1) and Alleged Victim (AV) engaged in an altercation that caused injury to AV. W1 and AV have a known history of resident to resident altercations with each other. An investigation determined that Alleged Perpetrator 2 (AP2) did not follow AV's and W1's behavioral plans when AP2 failed to provide redirection as an intervention to prevent the altercation between AV and W1. Facility documentation shows that AP2 received training for W1 and AV's behavior plans. AP2's failure to follow W1 and AV's behavioral plan is considered neglect of care and constitutes abuse. The facility is responsible for the supervision, training, and overall conduct of staff when staff are acting within the scope of his or employment status. The facility failed to provide a safe environment which is a violation of resident rights, is considered neglect of care, and constitutes abuse.
