Findings: The facility failed to follow the care plan for the Alleged Victim to use his/her walker while ambulating. On or about February 23, 2020, staff saw that AV was not using his/her walker and allowed him/her to continue without correcting him/her. AV fell a, causing injury to his/her head and eye. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
