Findings: On or about March 1, 2020 the Alleged Victim (AV) and Witness #1 (W1) were in the dining room and were involved in an altercation where W1 hit AV in the face. The care plans state to keep AV and W1 apart and staff are to supervise the dining room during meals to ensure resident safety. The facility's failure to provide a safe environment and follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
