Findings: Alleged Victim (AV) is a high fall risk. On or about July 1, 2019, Alleged Perpetrator 2 (AP2) used two gait belts, linked together, to limit AV's movement in bed causing AV unreasonable discomfort. AP2 stated he/she was in a hurry, was worried about AV falling out of bed, and could not get direction from other staff who were busy. AP2 did not know what a gait belt was used for and did not review AV's service plan prior to providing care to AV. The facility failed to ensure a qualified caregiver was providing care, which is a violation of resident rights, is neglect of care and constitutes abuse.
