Findings: The facility failed to follow the Alleged Victim's (AV) and Witness #1’s care plans. AV and W1 had an altercation when W1 entered AV's room. AV's room was care planned to be locked and was not, and W1 was care planned to be redirected from AV's room. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
