Findings: On or about March 20, 2020, AV fell while in the bathroom and hit his/her head which caused bleeding and AV was transported to the hospital for treatment. An investigation determined that AV showed signs of weakness, coughing, and having panic attacks prior to this incident according to facility staff. AV showed signs of having a change of condition prior to the incident; however, AV's care plan was not updated to reflect AV's change of condition. The facility's failure to properly plan care for AV's change of condition is a violation of resident rights, is considered neglect of care and constitutes abuse.
