Findings: The facility failed to follow the Alleged Victim's (AV) care plan to keep his/her door locked to keep residents from entering his/her room. On or about April 21, 2020, AV and W1 had an altercation when W1 entered AV's room, which resulted in a large skin tear on AV's arm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
