Findings: Witness #1 (W1) was to be monitored for behaviors, as he/she has a tendency to enter others rooms. On or about July 4, 2020, W1 entered the Alleged Victim's (AV) room and pushed him/her to the ground. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
